This will be a two-night camping trip.

Drop-Off: Tuesday Morning, July 8, at 6970 Canada Way

Pick-Up: Thursday Evening, July 10, at 7768 Wedgewood St

In the event I cannot be reached, I hereby give permission to the physician selected by the Pastor or adult chaperone to hospitalize, secure proper treatment, order injection, anesthetize, or perform surgery deemed necessary form my child.
